Final Forms

If you haven't done so already, please take the time to sign you son or daughter up for athletics.
 
  1. Log-in to your child's Final Forms account: https://columbus-oh.finalforms.com/.
  2. Click on Mifflin HS.
  3. Click on ALL 2026-27 sports that your child will participate in or try out for this coming school year.
  4. First official day of fall sports practice: Football August 1st. All other sports will be Monday, August 3rd.

The flyer also highlights a save-the-date for Saturday, August 1, from 10:30 a.m.

 

To be Eligible to Try Out, Students Must:

  1. Register for and complete final forms beginning for the 2026-27 school year: Go to http://columbus-oh.finalforms.com/ to register to participate in Mifflin Athletics. Head coach will than be able to communicate with you.
     
  2. Have a current athletic physical on file with the Athletic department.
     
  3. Pass 5 one-credit courses in the 4th grading period and have had a 2.0 GPA during the 4th grading period to participate. Incoming 9th graders must have passed 4 subjects and have had a 2.0 GPA during the 4th grading period of the 8th grade year Contact head coaches for sport specific questions.
